Título

Comorbidities and Severity Scores as Key Predictors of Outcomes in Invasively Ventilated ICU Patients

Objetivo

To comprehensively analyze the impact of comorbidities and severity scores on the outcomes of patients admitted to a tertiary ICU under invasive ventilatory support.

Métodos

A cohort study involving patients admitted in 2023 to a private ICU in João Pessoa, Brazil, under invasive ventilatory support.

Resultados

A total of 140 patients were admitted with invasive ventilatory support, 52.7% female, 64.3% medical patients, primarily from the Emergency Department (41.4%). The median age was 71 (52-83) years, 69.3% were frail, 56.4% required vasoactive drugs at admission, and 77.9% had a central venous catheter inserted within the first hour. The initial SOFA score was 8 (5-11), MFI score 0.09 (0-0.27), Charlson Comorbidity Index (CCI) 0 (0-2), and SAPS3 score 46 (37-55.75). Eight patients (5.7%) required tracheostomy, and 10 (7.1%) died. Patients who died had higher CCI 2.5 (0.75-4) vs. 0 (0-2), p=0.013, and SAPS3 66.5 (51.75-77.75) vs. 45 (37-53), p<0.001. The SAPS3 score had an odds ratio of 1.114 (95% CI 1.012-1.180, p<0.001) and the CCI 1.308 (95% CI 1.030-1.662, p=0.028).

Conclusão

Comorbidities and higher severity scores are significant predictors of mortality in invasively ventilated ICU patients. Each 1-point increase in CCI raised mortality risk by 30%, while each 1-point increase in SAPS3 raised it by 11.4%.
