Título

Liver Transplantation in Paraíba: ICU Profiles and Evolutionary Trajectories

Objetivo

To evaluate the profile, outcomes, and prognostic factors of patients admitted to a private ICU in João Pessoa after liver transplantation.

Métodos

A retrospective cohort study involving liver transplants performed at a private hospital in João Pessoa in 2023.

Resultados

6 liver transplants were followed, with patients aged 62 (35-70.25) years, 50% female. Key metrics included bilirubin 3.3 (2-6.9) mg/dL, lactate 5.2 (1.17-13.35) mmol/L, MFI score 0.22 (0.09-0.31), Charlson Comorbidity Index (CCI) 0.5 (0-3.25), SAPS3 52.5 (44.5-71), and EPM 8.99 (2.55-24.7). The median ICU stay was 3.5 (2.5-5.5) days, with a hospital stay of 8.5 (4.75-24) days. There was one death (mortality rate 16.7%). Factors associated with mortality included CCI (OR 1.271, 95% CI 1.183-1.365, p<0.001), SAPS3 (OR 1.123, 95% CI 1.1-1.147, p<0.005), MFI score (OR 1.43, 95% CI 1.295-1.579, per 0.1-point increase), and EPM (OR 1.065, 95% CI 1.055-1.074, p<0.001). The deceased patient had higher SAPS3 (77 vs 43), EPM (49.93 vs 1.35), CCI (3 vs 0), and MFI score (0.45 vs 0.09) than the medians.

Conclusão

In this small cohort, post-liver transplant patients exhibited high severity, with elevated initial lactate levels. Increased frailty (MFI), comorbidities (CCI), and severity scores (SAPS3, EPM) were significant mortality risks, highlighting the critical importance of these factors in prognosis.
